I have mod_security failure detection off, but sites are still being blocked.

It is set currently as:

LF_MODSEC_PERM = 0


Issue is occurring on 10+ servers.

That option doesn't enable/disable mod_security checking by lfd, that is the option to choose between permanent or temporary blocks. You should set LF_MODSEC to 0 and then restart lfd.
