Where do this spam go if you choose "forward to spam@domain.com"?

Do I need to create this mail account? Can we do this on a per e-mail account basis (it doesn't appear to be possible)?

You need to create the mailbox manually in cPanel. It is not possible to use the front-end to do this on a per-account basis, only a per-domain basis.

Is it possible to change this forwarding address to the default cpanel address? Rather then having to set up a spam address for each account.

You would need to change it to the full default account email address manually for each user, i.e. "username@hostname" instead of username@domain.
